CSA 1996 Gives Back: a Health Care Project

Objective:

To engage CSA 1996 alumni in a medical tele-consultation activity to help fellow Augustinians (batch mates, immediate family, CSA staff)

SMART: -

Specific — Use tele-consultation as a means to determine the concern and to deliver the prescription and/or treatment recommendations. -

Measurable — All interested patients will fill-out a Google Form. -

Achievable — Patients will be triaged to the proper medical specialist. Patients are defined as batch mates, immediate family, CSA staff. -

Realistic — Only a maximum of 4 patients will be given to one medical specialist for the duration of the activity. -

Time-based — The activity will run for 4 weeks (August 1 to 31, 2021)

CSA '96 ConSultA Program Mechanics

1. All CSA 1996 medical specialists (dentists, doctors, nurse practitioners, physical therapists, occupational therapists, speech therapists) will be invited to participate in the activity. All medical specialists will be categorized to allow efficient triaging.

2. A Google Form will be created to invite patients for consultation. - Name - Contact details: mobile number & email address - Current location of patient (Medical practitioners are limited by the area where they are licensed to practice.) - Concern (categorize: dental, medical (specify), rehabilitation (specify), others) - Google Form should also specify that patients will be triaged to the proper specialist and only 2 consultations (baseline & follow-up) can be accommodated free of charge.

3. Once patient is received (via Google Form), he/she will be triaged by the triage officer to the proper medical specialist. When there is more than 1 medical specialist in a given category, the patients will be assigned round-robin. Only a maximum of 4 patients per medical specialist will be accommodated.

4. Once patient is triaged to the proper specialist, the specialist will contact the patient regarding instructions on the medical specialist’s preferred mode of tele-consultation. A baseline and follow-up consultation will be set by the medical specialist free of charge. Beyond the follow-up consultation, the medical specialist has the prerogative to charge a professional fee accordingly.

ConSultA 96 Signing Out!

On behalf of the volunteer healthcare practitioners of CSA Batch 1996, today, August 31, is the last day of ConSultA 96 — our free telemedicine service.


It was an honor to serve the CSA community and give back to our alma mater. We are grateful for the success of our virtual homecoming and for being able to reconnect with our batchmates and former teachers.
